A. A water profile for the bathing area was prepared, containing: 1). A description of the physical, hydrological, and geographical characteristics of the water in the bathing area located on the river: a) elevation above sea level: <200 m, b) river watershed area: 1,000 km² or more, but less than 10,000 km², c) Abiotic watercourse type: lowland sandy-clay river—type code 19 2). Description of the physical, hydrological, and geographical characteristics of the water in a bathing area located on an artificial water body situated on flowing waters: a) elevation above sea level <200 m; b) catchment area of the water body: <10 km², c) Reservoir surface area at normal impoundment level (NPP): 0.025 km², d) Reservoir depth at normal impoundment level (NPP): max: 2 m; B. Public notification: flying a flag, websites—WSSE, PSSE, and bulletin boards located in the area adjacent to the bathing site; C. Identification and assessment of the causes of water quality pollution at bathing sites: a) The Municipal Public Utilities and Housing Authority in Lubaczów holds a water permit granted by decision of the Lubaczów County Administrator dated October 10, 2016, ref. no.: R.6341.67.2016, along with an amendment dated February 28, 2017, ref. no.: R.6341.10.2017, for: 1. the discharge into the Lubaczówka River at km 58+745, right bank, treated municipal wastewater in the following quantities: average daily flow Qavg = 2,660.0 m³/day, maximum hourly flow Qmaxh = 221.7 m³/h, maximum annual flow Qmaxa = 970,900 m³/year. 2. Discharge of municipal wastewater from the storm sewer overflow of the Lubaczów municipal sewer system via the storm sewer outlet into the Lubaczówka River at km 58+762, in a quantity not exceeding 10 discharges per year. Geographic coordinates of the municipal sewer outfall: N = 50°09’04.05” E = 23°07’28.06” Geographic coordinates of the storm sewer outlet: N = 50°09’04.23” E = 23°07’29.03”, 2. The Padre Pio Catholic Charitable Association in Jarosław, the Non-Public Healthcare Facility—Preventive and-Rehabilitation Center in Radawa holds a water permit granted by decision of the Director of the Przemyśl Watershed Management Board of the State Water Management Authority “Wody Polskie” dated August 20, 2020, ref. no.: RZ.ZUZ.3.4210.212.2020.MP, for water services involving the discharge of treated wastewater from the wastewater treatment plant located on the premises of the Prevention and Rehabilitation Center in Radawa, in the municipality of Wiązownica, into the Lubaczówka River at km 12+075 (right bank), in the following quantities: Qmaxs = 0.000244 m³/s, Qavg.d = 15.6 m³/day, Qmax.year = 5,694.0 m³/year. The wastewater outlet is located downstream of the swimming area in Radawa. b) Industrial wastewater treatment plant: Zakład Wyrobów Galanteryjnych Sp. z o.o. in Lubaczów holds a water law permit granted by decision of the Lubaczów County Administrator dated August 1, Zakład Wyrobów Galanteryjnych Sp. z o.o. in Lubaczów holds a water permit granted by decision of the Lubaczów County Administrator dated August 30, 2017, ref. no.: R.6341.69.2017 for: 1. the discharge of treated industrial wastewater into the Lubaczówka River at km 59+950 on the right bank, during the dry season, in the following quantities: maximum hourly flow Qmaxh = 3.5 m³/h, average daily flow Qavg = 30.0 m³/day, and maximum annual flow Qmaxa = 6,600.0 m³/year. 2. Discharge of treated industrial wastewater, as well as stormwater and snowmelt, into the Lubaczówka River at km 59+950 on the right bank during the rainy season, in the following quantities: Qmaxh = 46.02 m³/h, including stormwater Qmaxh = 42.52 m³/h, Qavg = 51.63 m³/day, of which stormwater Qavg = 21.63 m³/day, Qmaxr = 11,955 m³/year, of which stormwater: Qmaxr = 5,355 m³/year Geographic coordinates of the sewer outlet: N = 50°09’27.46” E = 23°08’10.05” 2. The Municipality of Lubaczów holds a water permit granted by decision of the Lubaczów County Administrator dated April 23, 2015, ref. no.: R.6341.November 2015, authorizing the discharge of washwater—treated in a washwater settling tank—into a watercourse constituting the old riverbed of the Lubaczówka River at a rate of 2.0 m³/day. The washwater is discharged from the groundwater treatment plant in Szczutków (Lubaczów Municipality). c) Illegal pollutant discharges—no pollutant discharges documented by the Regional Inspectorate for Environmental Protection (WIOŚ) d) Cooling water discharges—none documented by the Regional Inspectorate for Environmental Protection (WIOŚ) e) Discharges of treated rainwater or meltwater from the sewer system—rainwater from Wyroby Galanteryjne Sp. z o.o. in Lubaczów, as noted above. f) Discharges of wastewater from mine dewatering – none documented by WIOŚ g) Assessment of the potential for cyanobacterial proliferation – none detected h) Assessment of the potential for macroalgal or phytoplankton proliferation - not applicable i) Short-term microbiological contamination caused by: uncontrolled wastewater discharges from summer cottages, individual households, heavy rainfall, and a large number of swimmers; D. Actions: bulletin boards, flags, websites, increased monitoring of the Lubaczówka River (inlet) and the bathing area, ongoing cleaning of the banks and water surface by bathing area staff, cleaning of the screens at the inlet, organoleptic inspection, and monitoring as part of internal controls; E.
